Before-and-After School Program Interest Survey

Dear Parents/Guardians,
The Lakehead District School Board, has obtained the services of the Centre de leadership et d’évaluation (CLÉ) to manage a survey regarding the interest of parents in having a Before-and-After School program offered in their child’s school community. This program would be provided through a partnership with third party child care providers (eg. licensed child care centres or authorized recreational/skill building program providers).
Programs would be fee-based by the provider and would be dependent on sufficient enrollment at each school site. The estimated fees for these would also vary depending on the provider and the number of children registered .
If you are interested in a Before-and-After School program for your child(ren), we would invite you to complete the interest survey by clicking on the following link:

Please complete the survey by no later than March 23, 2018.
If you do not have access to a computer, please note that your child’s school will be happy to assist you in completing the survey. We would ask that you simply go to the school and ask the administrative staff for assistance.
This survey does NOT impact your current child care services and if you require services you need to register with The One List at : https://thunderbay.onehsn.com/.
The answers collected will only be used for the above mentioned purpose and in accordance with the Lakehead District School Board and CLÉ’s Code of Ethics which agree to respect the confidential nature of the collected data. Please note that answers/data will be collected by the CLÉ and provided to the Lakehead District School Board.
All comments left in the surveys presenting a risk of identifying a person will be paraphrased, or excluded, if required.
Also be advised that this is an interest survey only, actual registration for the program will occur if there is sufficient interest. You will be contacted should a Before-and-After school program begin in your specific school community.
